As a RN Case Manager , you will:
Admits patients to the services, completing appropriate clinical documentation under the Clinical Manager's direction. Assesses individual status and care/instruction needed by the patient (>50% direct patient care). Administers on-going care and case management for each patient, provides necessary follow-up as directed by the Clinical Manager. Confers with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provides hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revises plan in consultation with physician based on ongoing assessments and as required by policy/regulation.

In addition, a Gentiva RN Case Manager : Has the autonomy to make individualized patient-centered decisions for optimal clinical care. Works with other highly skilled clinicians through a multi- or interdisciplinary team approach. Qualifications

Diploma, Associate or Baccalaureate Degree in Nursing Six months experience in area of specialty/desired practice.

Current unrestricted licensure to practice as a Registered Nurse in the state(s) to which assigned and current CPR certification required.

Successful completion of any/all Company required pre-employment competency testing and health clearance prior to assignment and periodically as required by Company policy and state regulations.

Good organizational and communication skills and the ability to function with minimal supervision also required.
