:
This position is responsible for collaborating with clinical service lines in carrying out performance! improvement projects/initiatives for the Franciscan Health System (FHS) to address/strengthen patient safety, utilization of clinical resources and achievement of desired clinical outcomes. An incumbent serves as project manager for a variety various management-level work teams, providing support, expertise and guidance in: 1) the application of accepted clinical quality improvement methodologies/tools; 2) data collection and statistical/operational analysis; 3) outcomes measurement; 4) risk assessment; 5) best practices research; 6) documenting and implementing process improvements; and 6) evaluating desired vs. actual clinical effectiveness of performance improvement efforts.
Work requires strong clinical nursing expertise and an understanding of patient care standards/practices and applicable regulatory requirements. Considerable knowledge of project management and quality improvement concepts, methodologies and tools, and of the importance of data to performan! ce improvement processes (e.g. establishing baseline, providin! g objective measurement, determining the degree of the problem, showing variation/causes of variation, identifying root causes, developing appropriate solutions, etc) is also required. An incumbent is accountable for the success of assigned projects. Duties performed involve weighing and evaluating many factors requiring judgment, analytical ability and problem-solving in dealing with complex data and situations.
Esssential Duties
:
Manages the work of performance improvement team projects (in collaboration with FHS leadership/management, medical staff, department staff); evaluates the resources needed to support objectives; identifies and schedules project deliverables, milestones and required tasks; establishes standards and procedures for project tracking, reporting, record-keeping and documentation; monitors progress on key quality objectives and indicators, leading towards achievement of goals; keeps appropriate parties apprised of overall pr! ogress and monitors project results for significant deviations; proposes workarounds and alternative actions to ensure that timelines and deliverables are met; reviews project deliverables to ensure they meet standards and objectives.
Identifies problematic issues relating to performance improvement projects, determines reasons and actions taken to resolve, and advises higher level authority; conveys corrective plan of action to interested parties and follows up periodically to determine current status/disposition.
Meets with FHS clinical leadership to identify and discuss performance improvement strategies and opportunities as they relate to FHS values and/or business objectives, clinical outcomes, enhancement of patient safety and related matters; analyzes and assesses clinical practices for opportunities to improve healthcare delivery; gains consensus as to performance improvement projects to be undertaken; assists in the development and evaluation of s! tandards, procedures, processes, and automated tools that support estab! lished goals and regulatory requirements
Provides ongoing consultation and education with respect to the application of continuous quality improvement principles, standards and tools; explains and applies accepted methodologies (e.g. identify desired outcomes; analyze current problem/process; collect/analyze relevant data; uncover root causes; develop performance/process improvement plan and implementation strategy; pilot testing, assess results); emphasizes the effectiveness and efficiency of data-based decision-making in performance improvement activities.
Participates in the development, revision and implementation of policies, standards, procedures and protocols related to performance improvement initiatives; coordinates and/or chairs selected committees and interdisciplinary teams; provides timely and accurate reports to appropriate committees and management staff with respect to performance improvement efforts.
Serves as point-person for int! ernal and external project communications; ensures that project milestones, completion dates, costs, risks, contingencies and other significant matters are communicated on a regular basis to team members and stakeholders; negotiates the resolution of issues with all stakeholders.
Monitors and evaluates the clinical effectiveness of performance improvement work through data management and analysis; prepares reports of analyses, findings or recommendations; may develop and deliver formal presentations to interested parties.
Maintains current knowledge of JCAHO standards, patient safety goals, and other regulatory requirements impacting matters within assigned scope of responsibility; ensures that these factors are considered and integrated into performance improvement activities.
Maintains current knowledge of performance improvement methodologies and clinical informatics through regular review of the literature, attending conferences, participation! in professional organizations, etc; identifies and validates evidence-! based best practices in clinical practice and patient safety to guide clinical areas in performance improvement activities; researches nature/scope of impact on existing activities and makes appropriate recommendations.
Serves as coordinator for the correlation and distribution of patient satisfaction survey data; serves as liaison between the department and the Spirit At Work initiative.
May assist in data abstraction and auditing activities as needed, collecting data elements from individual records and abstracting clinical data on required elements from patient care records onto an established form following complex instructions on qualifiers for data elements to extract and exclusions; identifies outlier values or unusual data encountered in the abstraction process for additional review.
Performs related duties as required.
Qualifications
Education
:
Bachelor's degree in nursing or other healthcare-related d! iscipline
Experience
:
Five years of related work experience, including experience in outcomes measurement, risk assessment, data analysis/reporting and management of clinical work teams.
License/Certification
:
Current licensure as a Registered Nurse by the Washington State Nursing Commission.
Current driver's license valid in the state of Washington or other evidence of equivalent mobility.
