TV, patient advocate, educational material, and documents on patient chart, as observed by RN. Ensures patient has proper identification band. Inventories clothing and valuables, and has patient sign or documents reason for inability to sign. Assists with accurate assessment of unit specific population.
Participates in collection of information to include biophysical, environmental, functional, nutritional, significant other and family needs. Immediately notifies RN of changes in patient condition.Maintains skin integrity by proper turning, positioning and other actions to prevent development of decubitus ulcers. Follows skin care guidelines. Follows turning schedule set up by RN.
Assists in meeting the nutritional needs of patient by seeing that food and nourishments are given to patient, trays set up and that patient is assisted with feeding if necessary.Provides for ADL & delivery of patien! t care based on patient care knowledge and scientific principles, as delegated by the RN.Provides teaching materials pertinent to disease process and documents. Reinforces patient teaching that has been initiated by the RN.Answers call lights promptly.Ensures a safe environment by keeping work and patient areas orderly and clear of obstacles. Uses proper body mechanics, transfer techniques, and equipment.Ensures a smooth patient discharge and transfer process by assisting RN as appropriate. Assist with patient preparation for discharge beginning at admission and throughout patient stay.
Assists RN in completion of transfer summaries, discharge instruction summaries, and interagency reports.Maintains knowledge of emergency response plans. Responds in an appropriate manner to all potential and actual emergency situations following code procedures.Documents all actions and observations. Charting is accurate, pertinent, legible, complete and timely.Demonstrates knowled! ge and proper use of restraints, fall and seizure precautions. Acts as ! a resource for alternatives to restraint use and fall prevention interventions.Demonstrates knowledge of federal and state regulations that affect area of practice as observed by team members.
Professional performance and behavior is consistent with the Law Relating to Nursing Assistants. Demonstrates competent practice. Satisfactorily completes yearly unit specific competency assessment. Is viewed by peers to be an expert in clinical practice.
Certified in unit specific advanced skills.Completes or contributes to required reports. Assists in collecting patient acuity data. Participates in unit specific QA project and provides documentation. Participates and follows through with one unit specific QA per year.Attends, contributes to, and documents 75% of staff meetings.
Attends, contributes to, and documents 90% of staff meetings annually.Supports and enforces infection control practices. Uses hand hygiene, standard transmission precautions and dem! onstrates consistent use of personal protective equipment. Measured by observation. Demonstrates economy in the use of all equipment and supplies.
Ensures that supplies are charged to the appropriate source. Communicates recommendations that result in cost containment.Consistently remains productive by offering to help coâ"workers, checks equipment, cleans area, makes frequent patient rounds, and offers to assist patients. Takes on additional unit duties. (Ex: Tracks equipment, committees or special projects)Supports philosophy and mission of Harrison Medical Center.
