Join our dynamic team at Interfaith Community Health Center as a Ryan Whi! te RN Care Manager working at our Bellingham clinic. This grant-funded, benefited position will work 32 hours per week providing nursing care as part of a multi-disciplinary health care team. Care is provided to patients of varying socioeconomic & cultural backgrounds in an outpatient setting, primarily within the Ryan White HIV/AIDS Program. This position is responsible for care team achievement of clinical performance goals through leadership, delegation, problem identification, and continuous process improvement. Job duties include case management, RN visits for acute and chronic medical conditions in collaboration with providers; patient education; phone and walk-in triage; and quality improvement activities. This position holds a leadership role for the achievement of clinical standards for the Ryan White Program. Experience treating patients with HIV/AIDS and experience working at a community health center or similar environment are preferred. Cultural competency, str! ong nursing skills, & computer competence are a must!
