This job is responsible for directing and coordinating members of the care team, focusing on th! e provision of individualized quality patient care consistent with organizational standards. Develops the patient plan of care in partnership with physicians, interdisciplinary teams, and patient/family. This position is responsible for providing care to inpatients receiving hemodialysis therapy. The RN provides care to patients with acute or chronic renal failure who may be hemodynamically unstable, who manifest fluid and electrolyte imbalances, and require complex management before, during, and after treatment. Treatment may be provided on site or at a contract facility.
Essential Duties:
Manages and coordinates the care of patients in a defined care delivery model, utilizing the nursing process, which includes assessment, planning, implementation and evaluation.
Supervises and/or provides all nursing care provided to an assigned patient population. Any delegated care is based on the abilities and limitations of care personnel and patient acuity. C! ontinually monitors the performance of the team members report! ing to the RN and adjusts the delivery of care or clinical technique appropriate to the patient.
Collaborates and communicates effectively with the interdisciplinary team.
Accountable for patient/family education in collaboration with interdisciplinary care team and patient/family.
Documents nursing care and patient status in accordance with the nursing process and standards of care.
Provides a safe environment and promotes quality patient care through adherence to established standards.
Identifies opportunities for and participates in performance improvement activities.
Demonstrates competent professional practice, and engages in opportunities for professional growth.
Contributes to the productivity and professional environment of the department.
Performs pre and post-hemodialysis assessment to include fluid and electrolyte imbalance, intradialytic physiologic changes, and vascular access patency.
Manages individualized treatment plan ea! ch hemodialysis.
May provide continuous cardiac monitoring, arrhythmia recognition and intervention.
Critically assesses complications of treatment and performs/directs the interventions.
Performs all aspects of a hemodialysis treatment to include establishing blood access using asceptic technique and standard precautions.
Assures completion of initial and annual Medicare required care plans, assessments, problem lists, and other essential documentation within the specified time lines.
Performs related duties as required.
Education:
Graduate of an accredited school of nursing. Bachelor of Science in Nursing is preferred.
Experience: One-year experience within the last three (3) years in an acute care setting preferred.
Previous acute nephrology nursing experience preferred. 1-2 years of experience in a critical care or emergency department setting required. If no previous nephrology nursing experience, must successfully! complete Nephrology residency program.
License/Certificati! on:
Current State Licensure as a Registered Nurse required. ACLS, CCRN, CEN, CDN or CNN prefered.
Health Care Provider BLS required
