About this job

Candidate must be RN/LPN and have Case Management experience - prefer CCM certification and SNF experience

Experience - 2 to 5 years case management experience - nursing degree - and willing to travel between facilities

Our facility provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

This position requires a successful track record of interaction and negotiation with insurance representatives, as well as all aspects of the patient care management including referral development, assessment of patient clinical needs, and coordination of the interdisciplinary plan of care.

Requires RN or LPN with 2 years medical case management experience. CCM preferred

Minimum three (3) years clinical experience and two (2) years experience in medical case management. Comprehensive knowledge of workers' compensation, insurance, and managed care required. Prior experience with an insurance company, private case management company, or HMO preferred.
