Description: Providence St. Mary Medical Center is at the momment seeking for Clinical Specialist - Medical home & Transition of care right now, this occupation will be placed in Washington. The medical home/transition of care pharmacist will work in a family practice clinic with 3 family practice physicians and 1 nurse practitioner. This pharmacist will also work closely with the hospital's Chief Medical Officer, nursing, and case management in the identification of high risk patients for readmission, providing clinical pharmacy and transition of care services. This pharmacist will work closely with the Clinical pharmacy supervisor/residency director in the development of policies and procedures, preceptorship of residents and students, and travel/educational opportunities.
