Description

Coordination of providing quality services from the point of access through discharge.

Effectuates t! he process of assessment and admission in collaboration with physicians, patient, families, multidisciplinary team members, referring parties and unit staff.

Gathers, monitors, and evaluates information for the purpose of treatment planning and utilization review.

Organizes, coordinates, secures, and integrates resources necessary to accomplish the goals of treatment and discharge planning in collaboration with treatment team.

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process.

Qualifications

Education:
Graduate of an School of Professional Nursing

Experience

:

Three years pertinent clinical experience in a! n acute care setting.

Licenses/Certificates: Registered Nursing License, State of Washington

Current BLS certification or obtained during the first week of orientation
