The Department of Social and Health Services, Aging and Long-Term Services Administration (ALTSA), Home & Community Services Division (HCS) is recruiting for one (1) permanent position that reports to the Vancouver HCS Office's Residential Unit. The Aging and Long-Term Services Administration (ALTSA) assists adults with disabling conditions due to aging, disease or accident. We also assist children and adults with developmental disabilities to gain ! access to the high quality, cost effective supports they need.! There are four organizational divisions within ADSA. This position functions independently and assures that clients receive appropriate quality services which meet their individual needs and recognize their capabilities and choices through timely and responsive assessment, service delivery, reassessment, and case management. This position plans, promotes, develops and provides long-term care services responsive to the needs of persons with disabilities and the elderly with priority attention to low-income individuals and families. This position conducts face-to face comprehensive assessments and reassessments at the clientâs residence (in-home, hospital, nursing home, residential facility, etc.) using a laptop computerized system to determine client level of care, including determining nursing facility level of care needs. This position develops initial and ongoing care plans which provide the support necessary to allow clients to remain or return to the community. This ! position assesses and provides case management to adults at high risk of institutionalization or to those adults whose need for case management is part of a service plan developed following an adult protective service or investigation.
Duties:
Duties include, but are not limited to: · Conduct and complete face-to face comprehensive assessments and reassessments at the clientâs residence (in-home, hospital, nursing home, residential facility, etc.) using a laptop computerized system to determine client level of care, including assessing and determining nursing facility level of care needs. · Develop initial and ongoing care plans which provide the support necessary to allow clients to remain in or return to the community from nursing facilities, hospitals, or other institutional care settings or offer an alternative to nursing home placement through MPC, COPES and other HCS programs. Case Management: · Provide case management services to clien! ts meeting criteria set forth in Aging and Disability Services Long Ter! m Care manuals and policy. · Assess and provide case management to adults at high risk of institutionalization or to those adults whose need for case management is part of a service plan developed following an adult protective service investigation. · Make referrals to other community resources. · Consult and staff cases with supervisor. · Authorize services payments and complete forms. · File case documents, draft letters and respond to administrative requests for reports or constituent responses. · Refer to the Community Nurse Consultant on Skin Observation Protocols (SOPâs) and Critical Indicators when triggered as per ADSA protocol. Consult with nurse as needed on complex medical issues/terminology. · Provide client advocacy, as well as consultation, networking, family support and crisis intervention. Coordinate with families, facilities, local office staff, other regional offices and community agencies regarding needs of clients served. · Instruct and educate! other professionals and the public about what services can be provided, including program service limitations.
Qualifications:
REQUIRED EDUCATION, EXPERIENCE, SKILLS AND ABILITIES: MUST HAVE : One year of paid experience completing comprehensive assessments and determining nursing facility level of care for the elderly and disabled adult population Plus one of the following (experience can overlap): One year as a Social Service Specialist 2 (formerly Social Worker 2) since July 1, 1988. OR A Master's degree in social services, human services, behavioral sciences, or an allied field and two years of paid social service experience equivalent to a Social Service Specialist 2. *To have your experience count as â paid â status, please either : 1. put the salary in the requested section of the âWork Experienceâ section, OR 2. clearly state that the experience you are describing in the âDutiesâ section of the âWork Experienceâ is PAID . ! OR A Bachelor's degree in social services, human services, behavioral s! ciences, or an allied field, and three years of paid social service experience performing functions equivalent to a Social Service Specialist 2. *To have your experience count as â paid â status, please either : 1. put the salary in the requested section of the âWork Experienceâ section, OR 2. clearly state that the experience you are describing in the âDutiesâ section of the âWork Experienceâ is PAID . OR For Promotion Only: A Bachelor's degree and three years of experience as a Caseworker 3, Social Worker 1A or B, Social Worker 2, Casework Supervisor Trainee, Casework Supervisor, Juvenile Rehabilitation Supervisor 1 or 2, or Juvenile Rehabilitation Counselor 2 in State service. NOTE : A two year Master's degree in one of the above fields that included a practicum** will be substituted for one year of paid social service experience. ** For Practicum consideration, your practicum must be identified in the âwork experienceâ section of your application and! labeled â Practicum â NOTE : Employees must successfully complete the formal training course sponsored by their division within one year of their appointment. NOTE : Equivalent social service experience would include the previous classes of Caseworker 3 or higher. DESIRED EDUCATION, EXPERIENCE, SKILLS AND ABILITIES: · Knowledge of Medicaid terminology, regulations, and policies. · Ability to analyze a variety of programmatic issues/problems and develop workable solutions · Ability to use and learn software applications (e.g. Word, Outlook, and Internet). · Knowledge of payment systems (MMIS, SSPS, etc.) and service authorization procedures · Must be able to complete assessments in the homes and residences of individuals. · Must be able to perform the physical requirements of this position, such as moving/transferring equipment (such as a laptop, files, manuals, etc). · Ability to communicate and comprehend, through oral and written forms of communication, with m! anagement, peers, clients, and the public. o Ability to follow oral and! written directives/instructions. o Ability and skill to interview utilizing an interpreter or those with a mental or physical impairment/disability. o Ability to compose professionally cogent and grammatically correct letters. · Ability to prioritize, organize workload and reprioritize tasks and assignments to meet workload expectations. · Ability to resolve complaints between persons and groups with diverse personalities and goals. · Ability to identify and evaluate social dynamics affecting client choice and decision making. · Possess word processing and data input (keyboarding) skills.
