JOB SUMMARY:
Coordinates care for Pacific Medical Centersâ Managed Care patient in order to assure cost effective, appropriate utilization of health care resources. Nurse Case Manager manages, directs, and coordinates! the entire spectrum of care for patient throughout a variety ! of settings, both inpatient and outpatient. This position assists physicians (internal and external) with management and coordination of care.
REQUIREMENTS:
- BSN (strongly preferred)
- Current Washington State Registered Nurse License
- Certified Case Manager preferred
- Minimum of five (5) years clinical experience
- Previous experience in utilization review/case management
- CPR or BLS certification within 6 months of hire preferred
- Computer skills; typing, email and word processing
- Listens and communicates effectively
- Organizes and prioritizes work and handles multiple tasks
- Researches data; solves problems and thinks critically
- Demonstrates application of customer service skills
- Ability to use an Electronic Medical Records (EMR) system preferred
- Bilingual preferred, fluency with written and spoken ! English required
- Ability to work in a culturally diverse environment
