Location: Walla Walla Washington

Description: Providence St. Mary Medical Center is looking of Clinical Specialist - Medical home & Transition of care right now, this vacancy will be placed in Washington. Detailed specification about this vacancy opportunity please read the description below. The medical home/transition of care pharmacist will work in a family practice clinic with 3 family practice physicians and 1 nurse practitioner. This pharmacist will also work closely with the hospital's Chief Medical Officer, nursing, and case management in the identification of high risk patients for readmission, providing clinical pharmacy and transition of care services.

This pharmacist will work closely with the Clinical pharmacy supervisor/residency director in the development of policies and procedures, preceptorship of residents and students, and travel/educational opportunities.

Job Requirements

Education, training, experience:
Pharmacy practice residency and/or ambulatory care residency; or equivalent experience. Board certification preferred.
