POSITION SUMMARY: The Home Health Patient Care Supervisor, under the direction of the agency Director, is responsible for the day-to-day operations of assigned Home Health staff including supervision of staff; administering site-specific budgets; developing programs that are consistent with department goals and objectives; ensuring compliance with all hospital and department policies and procedures; and providing clinical support during periods of high patient census. EDUCATION REQUIRED: BS/BA or BSN is preferred, as applicable to Nursing, physical therapy, Occupational therapy or Speech Language Pathology or a combo of education and directly related work experience.
LICENSE / CERTIFICATION / REGISTRATIONS REQUIRED:
Must maintain state required licensure. Must have BLS certification Maintains driver's license in good standing and all required automobile insurance coverage. Reliable transportation
Continuing education and/or licensing requirements:
Must comply with state and professional continuing education requirements for continued license/certifications. Meet HHH Continuing education requirements.
Population Specific continuing competency requirements:
Must have documented age-specific competencies in all ages to which services are provided.
EXPERIENCE REQUIRED: Minimum of four (4) years experience in a treatment setting similar to the setting being supervised. Minimum one (1) years of managerial experience preferred.
Demonstrated skills in accountability, decision making, time management, oral and written communicaiton and interpersonal skills.

ACCOUNTABILITIES AND ESSENTIAL JOB FUNCTIONS:Program supervision:
Supervises all technical/clinical aspects of rehab performance and function:
- Plans appropriate staffing levels based on census and level of care needs. Develops contingency plans for unusual operating situations
- Monitors the workload of staff and makes adjustments when necessary to ensure admissions and visits are completed timely.
- Facilitates and promotes effective communication, teamwork, and cooperation within the rehabilitation department staff and throughout the home health team.
- Ensures that care is thoroughly documented and medical records reflect delivery of care in accordance with regulations, agency policy and procedure, and discipline best practice as adopted by agency. As appropriate, home bound status and the need for skilled care is clearly documented each visit.
- Outcome measures reflect a high quality of care delivered consistently compliant with Medicare COPs, state & federal law, Joint Commission standards, Hospital policy and Home Health Policy and Procedures.
- Care is delivered and documentation completed and submitted consistent with regulatory, accreditation, and legal requirements and with policy, procedure, and protocol established by HMC and HHH.
- Staff orientation: accountable for planning and execution of orientation of new team members. Takes initiative for development of new format or new training materials for orientation process.
- Supervises staff to ensure work is performed within prescribed technical and safety standards;
- Provides ongoing evaluation job performance to staff; provides education and guidance when required to address performance issues or concerns; initiates timely coaching/counseling/ discipline for repeated or intentional non-compliance with policy, procedure, assignments, directives, or regulatory requirements;
- Identifies behaviors and conduct that are disruptive or counter-productive and addresses immediately. Provides appropriate counseling; maintains complete and accurate records of counseling and discipline;
- Ensures all staff attends/completes all required training/education within timelines indicated and that audits or other assigned task are completed and returned within established timeframes. Tracks & trends individual compliance with regulatory and policy/procedure to identify individual needs for additional training/coaching and to capture objective, evaluative date for inclusion in performance evaluation;
- Maintains accurate time & attendance records;
- Works collaboratively and proactively with other supervisors/managers to ensure success across all areas of the organization including sharing of information, resources, and skills.
- When providing direct patient care, demonstrates the highest level of clinical competency, documentation skills, and professional behaviors described in the discipline specific job descriptions.
- Participates in the preparation and administration of department's staffing, supply and expense budgets. Monitors utilizations and adheres to labor and expense budgets. Is frugal and judicious in resource utilization.
- Develops and maintains a working knowledge of Medicare Conditions of Participation; State and Federal laws; Joint Commission standards; and Standards of Practice for disciplines supervised.
